Design, implement, monitor, evaluate, and modify or recommend modifications in drug therapy in consultation with the physician to ensure effective, safe and economical patient care.

1. Evaluation of patient data, scientific literature and pharmaceutical products in order to provide safe and effective treatment.
2. Optimization of drug utilization for therapeutic and diagnostic purposes.
3. Monitor the safety and efficacy of therapeutic plans.
4. Prevention or resolve medication related problems.

Identify, assess and solve medication-related problems, and provide a clinical judgment as to the continuing effectiveness of individualized therapeutic plans and intended therapeutic outcomes. Student shall be able to understand the clinical trials. The student is able to discuss disease pathophysiology and explain how subjective and objective findings correlate to pathophysiology for diseases commonly and routinely encountered on the rotation. Continuous self-learning to keep their knowledge and skill up to date through continuous professional development.
Identify pertinent laboratory data or testing that is required to assess efficacy/toxicity of current drug therapy or to evaluate new complaints. Student shall be able to appreciate the importance of adverse drug reactions. The student is able to develop and implement (when possible) a monitoring plan for the treatment recommendation(s) that will evaluate drug toxicity. Communicating with patients regarding disease, the drug treatment and alternative options to obtain informed consent and respecting patients’ own views and wishes in relation to drug treatment.
Evaluate patients and order medications and/ or laboratory tests in accordance with established standards or practice. Student shall be able to understand the scope of drug therapy monitoring. The student is able to correlate the gained knowledge to form the basis for rational use of drugs in a given clinical situation. Monitor and counsel patients regarding the purposes, uses and effects of their medications and related therapy. Student shall be able to provide the patient counselling services for rational drug use. Communicate and collaborate with health care professionals and patients regarding rational drug therapy, wellness and health promotion. Develop interpersonal and group skills and reinforce these skills by work in groups in case study presentations and practical laboratory.
Recommend pharmacologic therapy for each of the problem(s) assessed that is likely to result in desirable health. Student shall be able to understand the responsibilities of clinical pharmacists. NIL Familiarity with computer-assisted learning materials.
